All nominations must be submitted on the MNA awards
nomination form.

Each nominee must provide written consent
at the time of nomination in order to be considered a
candidate for an award.

The nominator(s)
must supply a current brief resume
of the nominee. It should include information on (a) staff council, (b) state activities, (c) national
activities, and (d) other leadership activities.

The nominator(s)
must provide a narrative statement, which can be written, videtaped or audiotaped, detailing the accomplishments
of the nominee. The statement should describe the nominees
compliance with the established criteria for the award
as explicitly and concisely as possible.

The nominator(s)
may supply letters of support describing how the nominee
meets award criteria. No more than two letters of support
will be considered.

Only requested materials will
be considered.

Materials received for award
consideration will be held confidential.

Members of the MNA Nominations Committee are not eligible to receive awards during
the period in which they serve.

All MNA members are eligible to be nominated for MNA awards.
- The deadline for submission
of nominations for MNA awards is July 15. Nominations
postmarked after that date will not be considered.
