Read your health plan disclosure form: This document sometimes called "evidence of coverage," is your road map of policies and rules. Know your coverage. Speak up: Be persistent in seeking the best health care from your health care plan. Do not accept a decision as final if it does not meet your needs or if you feel you need additional treatment. Ask your primary care practitioner to describe treatment options. Be your own advocate. Keep your membership card handy: You will need your card when you visit your primary care practitioner, have medical tests, purchase prescription drugs, or access emergency care. Know the phone number for member services: Health plan representatives are available to explain or answer questions about your coverage. Keep written records of: all correspondence with the plan, claim
forms and copies of bills, phone conversations - the date and time,
the people you speak with, and the nature of each call. |
